Environmental Levy Scheme on Plastic Shopping Bags - Registration

Change of Information

Notification of change of information

If a change in the information provided to the EPD in relation to an application for registration occurs after it has been approved, the registered retailer must notify EPD through the submission of the "Notification of Change of Information" form (Form PSB-2). The changes of information that require notification include:

For registered retailer

Change of address of registered office
Change of name of registered retailer
Change of proprietor of registered retailer
Change of partner of registered retailer

For registered retail outlet

Change of business address
Change of name of retail outlet
Change of franchisee

Revoke Franchisee

When to submit?

The registered retailer must notify the EPD by submitting the completed "Notification of Change of Information" form as soon as practicable but in any case within 30 days of the change.

The completed "Notification of Change Form" should be submitted by post to:

Failure to submit notification on time

A registered retailer who, without reasonable excuse, fails to submit the notification within 30 days after its occurrence commits an offence and is liable on conviction to a maximum fine of $5,000.
