Part A1 (Particulars of retailer – body corporate application)

• This part should be completed if the applicant is a body corporate.  It is not applicable to individual or partnership application.
   
• A copy of the valid Business Registration Certificate of the retailer must be submitted together with the application for verification.

Part A2 (Particulars of retailer – individual application)

• This part should be completed if the applicant is an individual.  It is not applicable to body corporate or partnership application.
   
•  A copy of the valid Business Registration Certificate of the retailer must be submitted together with the application for verification.
   
• A copy of the valid Hong Kong Identity Card of the proprietor must be submitted.

Part A3 (Particulars of retailer – partnership application) 

• This part should be completed if the applicant is a partnership application. It is not applicable to body corporate or individual application.
   
• A copy of the valid Business Registration Certificate of the retailer must be submitted together with the application for verification.
   
• A copy of the valid Hong Kong Identity Card of each of the partners must also be submitted, where applicable.

Part B1 (Particulars of retail outlet) 

•  This part should be completed by all applicants.
   
• Please use a separate Part B1 for each retail outlet. For example, if a prescribed retailer wishes to register 5 retail outlets, he would need to use 5 separate Part B1 to fill in the required information..

Part B2 (Particulars of franchisee)

• This is an optional item and is required only if the retail business is carried on under a franchise arrangement.
   
• Please use separate Part B2 for each retail outlet.
   
• If the franchisee is an individual, a copy of the franchisee’s valid Hong Kong Identity Card must be submitted together with the application for verification.
   
• If the franchisee is a body corporate, a copy of the franchisee's valid Business Registration Certificate must be submitted together with the application for verification.

Part B3 (Application for Type 1 exemption)

• This is an optional item and is required only for application for exemption of part of the area of the registered retail outlet.
   
• Please use separate Part B3 for each retail outlet. 
   
• To be qualified for Type 1 exemption, the following criteria must be fulfilled:
   
1. the total floor area of the retail outlet in which only non-specified goods are displayed or offered for sale is more than 50% of the retail floor area of the entire retail outlet; and
    
2. each part of the area to be exempted is a cashier’s counter that accepts payment of non-specified goods only (i.e. goods other than food or drink; medicine or first-aid item; and personal hygiene or beauty product) 
    
• Information required for Type 1 exemption application:  
   
1. Total retail floor area in which only non-specified goods are displayed or offered for sale;
    
2. Retail floor area of the entire retail outlet; and
    
3. The floor plan for each level of the retail outlet specifying the following information :
    

• Any area in which only non-specified goods are displayed or offered for sale;
   
• The identification number and location of each cashier’s counter that is intended to be exempted;
   
• The scale of the floor plan;
   
• The floor plan identification number;
   
• The corresponding floor level number;
   
• The retail floor area in which only non-specified goods are displayed or offered for sale; and
   
• The total floor area of that floor.

Part B4 (Application for Type 2 exemption)

• This is an optional item and is required only if the applicant intends to apply for the exemption of part of the area of the retail outlet that is used for the purposes of a third party’s business.
   
•  Please use separate Part B4 for each retail outlet.
   
• To be qualified for Type 2 exemption, the following criteria must be fulfilled:
   
• the part of the area of the retail outlet to be exempted is used for the purposes of a business of a third party (not being the registered retailer) registered under the Business Registration Ordinance (Cap. 310);
   
• that area is demarcated clearly by a counter and is used exclusively for the purposes of the third party’s business;
   
• the name, trade name, trade mark or brand name of the third party is displayed conspicuously in that area;
   
• that area is staffed by an employee of the third party; and
   
• all plastic shopping bags provided from that area bear the name, trade name, trade mark or brand name of the third party.
   
• Information required for Type 2 exemption application: A floor plan of the retail outlet indicating which part of the area of the retail outlet is occupied by the third party and is intended to be exempted.

Part C (Declaration)

• This part should be completed by all applicants.
   
• In addition, Section C(1) should be completed for application by a body corporate; Section C(2) should be completed for application by an individual; Section C(3) should be completed for application by a partnership.
