applicant is preferably the main contractor
of the construction site concerned.
If
the applicant is a joint venture or consortium
company, please provide the names of all the
concerned incorporated mother companies.
If
the applicant is a joint venture or consortium

site (such as land lot number, street number,
and nearby landmark), and project name, if

provide a plan of appropriate scale (preferably
1:1000) just enough to show a clearly marked
boundary of the work area where there are
unavoidable constraints on working hours and
the locations of any nearby noise sensitive
receivers. An updated survey sheet issued

If
the application involves powered mechanical
equipment at fixed locations, relative distances
of such equipment to the site boundary or
grid lines shall be indicated in metres
on the submitted plan(s).
For
cases involving noise correction(s) due
to topographical features (e.g. underground
work, complicated terrain, and high-rise
building), please provide a scaled plan
and sectional drawings showing the screening
effects of such features between the noise
source(s) and the NSR(s).

in the application would be used in connection
